If you are an in-center hemodialysis patient at ºìÌÒÊÓƵ¸ßÇåappÏÂÔØ, you may receive the Medicare In-Center Hemodialysis (ICH CAHPS) Survey through the mail or a phone call from our survey vendor, Press Ganey. The Center for Medicare & Medicaid Services (CMS) has chosen a sample of eligible patients to participate in the survey. Please take a moment to share your experience and help ºìÌÒÊÓƵ¸ßÇåappÏÂÔØ improve our patient care.

You may also receive a phone call from Press Ganey.
Your participation is completely voluntary. All answers are protected by the Privacy Act and no one will be able to connect your answers to your name. Your answers will help us improve the quality of care at your center.
